WHY! why apply hydroxyl cut to hypertensive patient???? THE ANSWER IS AS BELOW

An hypertensive patient was dispensed by his pharmacist with diovan ( consisting of thiazide, valsartan, ACE inhibitor ) together with hydroxyl cut( an energy booster and weight lost agent), active ingredient of his hydroxyl cut is made up of aspirin (to thin the blood), caffein(increasing body metabolism rate including catabolism of lipid) , white tea extract(to reduce the production of fat storage cells), and garcinia cambogia(to reduce appetite).


Thiazide is a relatively safe diuretic to be used since it has a broad therapeutic index. This meant that in normal dose and even in the case when some deviation in dose occur accidentally, thiazide still can work relatively effective without causing toxicity. But currently, the research shown that thiazide can cause metabolism impairment after long term use, the patient is at risk of getting diabetes mellitus,hypercholesterolaemia as well as hypokalaemia(excessive loss of potassium ions ), hyperuricemia..... So a new drug is actually been discovered named indapamide  (better drug,expensive not many can afford it), which bring little side effect on glucose excretion and uric acid excretion. But since our patient is recently taking thiazide together with ACE inhibitor (having the effect to help retaining potassium lost), hypokalaemia is not a problem for him  . And for the metabolism impairment disease (diabetes mellitus,hypercholesterolaemia ) caused by long time use of thiazide, there is also no need for our patient worry, because he is recently taking hydroxyl cut which helps him to improve body metabolism and reduce lipid level in blood, reducing his risk of getting disease like hyperglycemia and hypercholesterol. 
    Since thiazide is harmles to kidney with little monitoring ,as it has large therapeutic index and does not cause accumuation of calcium ions in urine (it happens only when loop diuretics is used ,bcz calcium oxalate can lead to kidney stone) there is no need for frequent kidney check up. BUt...i repeat 'BUT' since the ACE inhibitor is part of his medication ,it is advisable that he goes for kidney check up consistently ,because although we said that thiazide is save for kidney but ACE inhibitor does bring harmful effects to ur kidneys.
   And one thing i would like to stress here is his pharmacist is really brilliant , hydroxyl cut and ACE inhibitor are used to counteract with his thiazide side effect . So he actually save a lot economically because there is no need for him to buy indapamide . But in long run, kidney failure can be his timing bomb, so regular check up for kidney is needed, prevention is better than cure. Ususally Glomerular filtration rate and creatine value are measured during diagnisis of kidney disease. ACE inhibitor is not remove by his pharmacist ,i think it is because thiazide alone is not effective enough in treating hypertension since combination of drug works better.(treating hypertension is more important because it not only leads to kidney failure, but other life threatening disease like stroke and heart problem).Conclusion, he should be thankful to his pharmacist. :D
